Request for Lab Services & Evidence Submission
Use this form to submit evidence to the Alaska Crime Lab and request specialized laboratory services. One form is required for every case submitted and should include all related evidence items. This form may also be used to request additional testing on existing cases. Complete all required forms before delivering or shipping evidence to the laboratory.

Every evidence submission must include a completed Request for Lab Services Form. Evidence received without this form may be returned or delayed.
Request for Lab Services
Primary form required for all physical evidence submitted to the ASCDL. Include agency name, case number, item descriptions, and specifically requested analyses. Submit one form per case — all items for the same case on the same form.

Sexual Assault Examination Kit Forms
Forms used by law enforcement and medical providers when completing Sexual Assault Examination Kits. Each victim type has both a fillable PDF (complete on screen) and a printable version (print and complete by hand). Use fillable versions whenever possible for legibility.

Victim Notification
Required notice to victims that their SAK will be submitted for forensic testing. Must be completed and provided to the victim prior to kit submission. Effective August 1, 2023.

DNA Forms
Forms related to voluntary DNA sample collection and CODIS database submissions. The Volunteer Form is used to collect reference samples from individuals who consent to providing a DNA sample. The DISC form is used to enable forensic DNA profiles in CODIS for individuals with special concern designations.
DNA Volunteer Sample Collection Form
Used when collecting a voluntary buccal swab reference sample from individuals who consent to providing a DNA sample to the ASCDL. Includes consent language and chain of custody section.

DNA Index of Special Concern
Used to enable forensic DNA profiles in the CODIS national database for individuals designated as persons of special concern. Requires law enforcement agency authorization. Contact the lab before submitting.
